Cancellation Policy

At Summerfield Custom Wellness, we respect that work and life demands and changes in your schedule are not always in your control and that situations arise when you may need to cancel or reschedule your appointment(s).  We kindly request as much notice as possible.  Please do your best to attend all scheduled appointments, provide at least 24-hour notice if you need to change an appointment, and take steps to avoid ‘no showing’ your provider.

After three late cancellations or missed appointments, patients are given the option to either sign a Cancellation Agreement in which a $35 Cancellation Fee is implemented or discontinue services.  When patients provide at least 24-hour notice, it is not considered a late cancellation, as we can offer that time slot to another patient.
